Soft focus - How to add soft focus to a photograph

To add a misty, soft-focus quality to a photograph, shoot the picture through a nylon stocking, or one leg from a pair of nylon tights, stretched tightly over the lens with a rubber band.

Or simply breathe on the lens just before taking the picture.

You could also spread petroleum jelly onto the filter. Wipe it off afterwards with a soft tissue.
